Documentation

Providers must prove that they have provided services through service documentation.

Documentation must be:

  • Stored in an accessible location
  • Provided, upon request, for review or an audit
  • Stored for six years from the date of payment or until the outcome of an audit, whichever is longer.

Providers must complete documentation and billing training within 60 days of being selected to provide services. This training is found on DODD's MyLearning website or through NEON. (See Resources for more information on training.)

Forms

DODD does not require providers to use one specific form to document services. However, there are requirements about what information must be documented. The requirements are stated in Rules specific to the services being provided. Click here to learn more about these Rules.

Cuyahoga DD created sample forms for common services. Please note: These forms are only to be used as a guide to assist providers in creating their documentation.

For required MUI monthly, annual, and incident documentation forms, visit the MUI page.
