Yes, you can apply for services on behalf of someone else. Cuyahoga DD reaches out to the family when we receive the application. We cannot process the application if we cannot connect with the family. You will need to check with the family for updates on the application status and their eligibility process. No, County Boards of DD do not have waiting lists for waivers. Waivers are given based on a person’s immediate needs. We use a common tool to assess these needs. Depending on the results, you may receive services or support through a waiver, local funding, or community resources.

Cuyahoga DD’s Early Intervention (EI) program helps children from birth to age two who have a developmental disability or delay. If you receive services funded by a Medicaid Waiver, your Support Administrator (SA) will help you find a caregiver and check on the services you receive.
If you want to find a caregiver on your own, you can use ProviderGuidePlus. This online tool helps you look up caregivers by service type, and you can also read reviews from other families.
It depends. The Family Supports Program is for people who qualify for Cuyahoga DD services and are not enrolled in a Medicaid waiver. Individuals living in an Intermediate Care Facility (ICF) or in foster care cannot participate. There are no income requirements. Eligible individuals can receive $750 each year. ICFs are facilities funded by Medicaid that provide a package of services for people who need a lot of support. These services include help with daily living skills like eating, dressing, hygiene, and taking medications, as well as day services, employment services, nursing, and therapies such as speech, occupational, and physical therapy.
