Cuyahoga DD values families' important role in supporting individuals with developmental disabilities. The Family Supports Program uses local tax revenue to help families access goods and services.

Eligibility 

The Family Supports Program is only available to people eligible for Cuyahoga DD services who live with family or on their own.

The following individuals are not eligible for the Family Supports Program:

  • People who are enrolled on a Medicaid Home and Community-Based Services Waiver* (Level 1, Individual Options, or Self-Empowered Life Funding)
  • People who live in an intermediate care facility (ICF)
  • People who live in foster care

*If you have a Support Administrator, please contact them. You may be eligible for similar services.

Annual funding allotments for people eligible for Cuyahoga DD services:

  • 17 and younger, $750-$1,500 per person
  • 18 and older, $1,500 per person
 
 
 

Respite Care 

Respite care gives families a short break when they need it. This type of care is for people who need specialized help that untrained providers may be unable to give.

There are two types of Respite Care Providers:

Certified Respite Provider: These providers are certified by the Ohio Department of Developmental Disabilities (DODD) as Waiver Providers. To get certified, they must pass a background check and complete training in courses about individuals with disabilities, as well as First Aid and CPR. They also need a signed contract with NEON. Below is a list of Certified Respite Providers in Cuyahoga County.

Family-Selected Provider: This provider is someone the family knows, like a family member, friend, or neighbor, and they believe this person can care for their family member. The family trains this provider. The provider cannot live in the same house as the person they care for, cannot be their parent, and must be at least 18 years old. You can complete the respite invoice or timesheet below.

Requesting Equipment 

Equipment requests are made through the online request form. Your occupational therapist, physical therapist, speech and language pathologist, or physician can complete the equipment request form below.
